Legal Question in Family Law in California

Hello,

I am a military spouse currently living in San Diego. My husband is in the navy and has recently been approved on permanent change of station ordres for our family to move to Jacksonville, Florida. My question is what form do I need to file with the San Diego Superior/Family Court to let them know that I am moving to Florida with my daughter? I have custody/child support orders with the San Diego Family Court that has granted me legal custody of my daughter and about 10% vistation with her biologoical father, who resides in Las Vegas, Nevada. The custody orders have California as the home state of my daughter and her primary residence resides with me too.

My husband and I are to depart to Florida on April 1, 11 and my daughter is to finish the rest of the school year with my parents here in San Diego. Once, the school year is over in July my daughter will be living in Jacksonville with me an my hsuband on his orders. Please let me know what form I should with the San Diego Family Court. Thank you!

What attorney Mataele doesnt' tell you is that you don't just "notify" the court you are moving. You need a court order ALLOWING you to move your daughter out of California. The forms listed in Mataele's answer are just the bare bones for obatining the order. There is no way a lay person could possibly fill them out in such a way as to be successful in obtaining a move-away order, without help. Move-away's are not the realm of amateurs or the faint of heart. You need to consult with an attoney. You don't necessarily need to hire one to represent you, but you will at least need guidance and advice. So talk to a few who offer "limited scope representation" about how to structure a relationship that will cost you the minimum in fees but make sure you get the result you need.
