Modification of Child Custody Agreement

My ex-husband and I have agreed that I am to have sole physical custody of our 14 year old daughter as she requested. Since we are in agreement on this, can I completed the required forms with the court myself? What forms should I ask for? I also want to request child support in the form of a wage garnishment since he says he won't give me a dime. Is this something I can also do myself?
