What steps do I take in order to make a modification of custody (from sole to joint) in which both parties agree upon? I s ther a form in this area?

If both parties agree, all you need is a stipulation and order that can be prepared by any exprienced paralegal assistant or family law lawyer.

There is no form, but you will need to prepare a stipulation with a provision for the judge to approve the agreement and make it an order. You might check with you court facilitator's office to see if they can prepare the form for you.
