modification of visitations

My fiancee has custody of his son and his mother is not making efforts to see him or support him she only pretends to care for him but is using him.We would like to modify the visitation schedule and possibly get a supervised visitation order but have no idea how to go about it?We do not want to make it seem like we are trying to take anything from her we know she is his mother but she doesn't act like it.She pays no child support, has no job,does drugs, is mentally ill how can we work drug tests to be a stipulation?Her boyfriend also has a violent temper as does she.

you can file an OSC to modify custody and support and also file an OSC re contempt for the unpaid support.
