Move a case

I have a divorce case with joint custody of our son in Stockton court. I want to move it to San Diego and then change it to sole custody. What do I need to do specifically to move my case to the San Diego courthouse?

It requires an NOTICE OF MOTION AND MOTION TO CHANGE VENUE, POINTS AND AUTHORITIES IN SUPPORT THEREOF, DECLARATION IN SUPPORT THEREOF, AND PROOF OF SERVICE filed in Stockton. A hearing will be set.
