Legal Question in Family Law in California

I have an �Order After Hearing� that states �Either party may make an ex parte application to the Court requesting the direct release of the evaluation.� What form do I use to file the Ex Parte Application in the Fresno (93721) Family Court system?

Unless there is a local form, which you can get from the court, there is no form. You have to prepare a regular ex parte motion.
