Parents using a surrogate mother

We want to have our names on the birth certificate of our child. Our sister-in-law is carrying the child and we have a written agreement. We cannot afford an attorney in this process. Can we go to the courts ourselves? What do we do to get a court order so the hospital will put our names on the birth certificate instead of the surrogate? Is there a county, state agency or a division of the district attorney's office that can help us?
