Legal Question in Family Law in California

I am preparing to have served on my ex-husband FL-485 Notice of Delinquency and the appropriate attachments. Do I file with the court first, then have him served and re-file to include the proof of service, or do I have him served without the court stamp and then file the entire completed form with the court and is there any fee for filing FL485? Additionally, page two asks for the complete contact information of the child for whom support was ordered (1991); however, she is now an adult. Am I required to divulge her contact information on this form, or can I indicate that she is now an adult? Thank you!

As I read the statutes governing the procedure, the form must first be completed, and filed with the court. It is then served in a manner provided by service of summons such as personally or certified mail.

The statutes also provide that you must provide the current address of the supported children.
