I private agreement child guardianship Fm code 6550

I need ''private agreement for child guardinship, this is covered in family code 6550 , I have not been able to find this form, can you help???

i have 12 year old boy live at my home during school time, (second year) Foster care says I have to be his legal guardian, this apears to take 5 to 7 weeks and cost up to $600.00.

In the Gurdinship pamphlet it sttes a private agreement can be drawn up, and this is in code 6550 which i cant find Can you heklp me ??--name removed--Keller

California's state web site has all of the code listed and you can obtain a copy there. Each county has a law library, and many local libraries have copies of the code sections. Section 6552 contains a sample agreement.
