Can Proof of Service Form, once served, be mailed or faxed back to court?

Very few courts accept direct fax filing. Alameda County is the only one I know of off the top of my head. You can always file any paper by mail as long as you have the time for it to get there before any applicable deadline. You will need to make sure you enclose as many copies as the court requires, which in the case of a POS should only be one, and then another copy or two to be endorsed filed and returned to you. Also enclose a self addressed return envelope with sufficient postage for the return copies.
