releasing 1/2 of 401k to ex spouse what forms need to be filled out?

That depends on how you are "transferring" it. Hopefully to an IRA or other IRS authorized account. Otherwise you are facing expensive tax consequences. If it is a straight 401k to IRA, SEP IRA, or other authorized account, you will just need to fill out whatever paperwork your 401k administrator requires. If you plan to release it in cash, you are making a big mistake and need to talk to a lawyer in person.
