request change in custody/visitation

Previous to my divorce being final, my mother had guardianship of my son, neither myself nor father had parental rights. When divorce papers where finally processed, physical custody was with the father. I've not been allowed visitation in last 12 months, now not even allowed phone contact. Where do I start a request for change in custody/visitation? Father is living in MA,

I live in El Dorado county CA

Divorce finalized in Contra Costa CA

Guardianship in Alameda CA
