Im the respondent in a divorce case. In my response to the petitioner i sent her form interrogatories. It has been six weeks and she has not responded. what form do i file with the court to have the case dismissed?

Woah, there cowboy. You don't get to have a divorce dismissed for being late answering interrogatories. Even in a civil lawsuit, where dismissal MIGHT be an available remedy in extremely rare circumstances, you don't get that order until after numerous prior orders to respond and respond more fully have been ignored.
I agree with Mr. McCormick. The simple answer is you don't. At this point you need to file a motion to compel responses to the form interrogatories. That motion is easy for an attorney to do, who has litigation experience. It is much harder for a lay person who does not have experience with civil litigation.
