Restoration of Maiden Name During Divorce Proceeding

I have not yet received the default judgement for my divorce, but I need to change my name back to my maiden name. What forms can I file to make this happen as quickly as possible? Also I live and filed for divorce in California.

File an OSC requesting the name change. IF there is an emergency reason for doing this, file ex parte.
