Resuming maiden name

My daughter was divorced in PA in 2005. She is living now in CA. She wants to resume her maiden name. Which state does she apply to for the name change? What kind of form?

She can apply for a name change in California. You will have to ask a PA lawyer if she can apply to change her name there.
