I have sole custody of my child, will still need her father's signature to request her last name change?

No, but he needs to be given notice of the hearing on a petition for a name change for his minor child. He has to be afforded the opportunity to object. The court makes a decision based on the best interest of the child.

You need to file a petition for a name change. This requires you to give notice to the father and to publish the petition in a local paper. The Court than has a hearing where father will have the opportunity to object. Good Luck.
