Can I use standard form to request ''Motion to dismiss/Health Reasons

I want the judge to dismiss all court pertaining to case due to spouses' health concerns. I'm the Respondent, she is really ill at this time, can the judge suspend or dismiss all orders? Restraining and visitation if we both approve? Which form can I submit to the court? Can I ask this question?
