Step-parent authorizes medical immunization

I have joint custody 50/50 of my son, and I just learned that my ex-husband's wife took my child in for a hepatitis shot without my knowledge or consent. Does a step-parent have any legal rights in this area? Has she broken any laws?

I expect that she took your child for the hepatitis shot at the father's request. I would be more concerned about why he was taken to the doctor for the hepatitis shot, than whether she violated any laws. Contact father and ask about why he was given this shot. After getting this information you can then determine the proper course of action.
