How do I subpoena documents ?

It has been suggested (see previous responses to my incessant queries) that I subpoena bank accounts statements from my ex-wife. How do I do that? Also, I would like to subpoena the record of installation of a phone number - I am not interested in phone records, just the date the phone number was designated and installed. How do I do that?

I suggest that you visit your local law library. Look in the phone book for the location of the county law library. Most law schools also have designated hours for the public. Ask the reference librarian for practice guides on evidence. Remember that you will need to learn not only how to get the evidence you want, but how to present it to the court in an admissible way.
