Substitutions for form FL-350

I have a pending divorce case in Los Angeles county, it has been rejected because the court requests form FL-350. I live in San Diego county, the divorce was filed over a year ago when I lived in LA, I also just received a order from San Diego Superior court for custody / visitation and a three year restraining order. Also, Los Angeles county DA filed for an order for child support in August, the default order is pending. I recieve public assistance in San Diego county, therefore they are in the process of taking jurisdiction over the child support issue.

I am desperate to obtain my divorce. Bottom line is the form required is very difficult to complete. One, the ex will not sign it, there is a restraining order against him. Two, there needs to be a signature from someone in the child support agency, neither county will sign because of either where I live or pending legal process on there end. Is there anyway to substite another document or declaration? What is the difference of filing for dissolution with status only? Are there any other options to having the court accept the default judgement I am asking for?
