Legal Question in Family Law in California

If I have sufficient evidence/document to show that a judge (California family law court) did not give me fair hearing (again and again) and that she made a custody decision that has harmed the children emotionally and left the children unsupervised and uncared for, how can I request change of judge? How can I file an official complaint?

You could have your lawyer appeal, if there are grounds for an appeal and if the legal time limit hasn't passed.
