Temporary to permanent child guardianship

What form or how would I give guardianship of my daughter (13) to my sister should something happen to me? I many health challenges and sometimes long hospital stays are necessary for my recovery. My spouse recently passed away and my daughter is adopted. I don't want her to go back to her birth mom (in mental institution) relatives.

You should consult with an attorney. However, as a general rule once a child is adopted the rights of the parents is terminated. That termination of parental rights is permanent, unless the court were to for some reason reinstate those rights. Most parents appoint a guardian for their children through a will.
