Terminating a Legal Separation

My wife and I obtained a Judgment of Legal Separation in San Francisco, CA on March 15, 1985 and now intend to reconcile.

The Judgment provides:

''In the event of a reconciliation between the parties hereto, it is mutually agreed, in consideration of the premises, that this Agreement, or any judgment incorporating this Agreement, shall stand and remain in full force and effet, unless expressly terminated, canceled or dismissed in writing by the parties.''

1. Is there a prescribed form that both parties must sign?

2. Must the parties file the form with the Court?

Does this mean that we must file such a document with the Court?
