Termination of Father's rights

I am divorced from my 8 year old daughters father. He is incarerated for molesting one of my older daughter. While he was in prison I divorced him, it was uncontested. He has been in for 3 years and has one and half years to go. We have heard nothing from him for 4 years, no child support or contact. I have raised my daughter alone. I was givin full and sole custody of her, without supervised visitation. NO visitation. I would like to terminate his parental rights, however I have not found a form to petition fo rhtis. I do know that he will not contest this either. Thank you for your help ion this matter.
