What type of subpoena or subpoena form may I use to have nurse assistants testify about my participation and my character for a conservatorship petition?

I hired an attorney who attempted to deceive me into agreeing to a co-conservatorship during a mediation (California). Therefore, I am pursuing the conservatorship legal procedure on my own. Furthermore, there are a few nurse assistants who are willing to speak in my favor; however, they are fearful of retaliation from their supervisor if they write any statements for me. (I made a complaint against the head nurse for poor care provided to my sister to the State Ombusperson). The conservatorship is being contested by a sibling.

I asked the local law library staff for possible written subpoenas types of forms for this, but to no avail. Below are the subpoena forms list on courtinfo.com. I would appreciate any advise or guidance possible. Thank you

The type of subpoena depends on what proceeding you will be conducting at which the testimony will be given, whether the witness will be required to bring documents, and whether or not the documents constitute "consmer records." So the information in your question is not adequate to answer it.

You would use the first one on your list to get them to come to the hearing -- Civil Subpoena for Personal Appearance At Trial or Hearing. You use the second one if you want them to bring some papers - - Civil Subpoena (Duces Tecum) for Personal Appearance and Production of Documents and Things at Trial or Hearing and Declaration.
