I usually file an affidavit of facts constituting contempt, and have the court issue an OSC re: contempt.

Contempt is a difficult process and the other party must be proven guilty beyond a reasonable doubt, with many issues of proof other than that they violated the court order. The most practical solution is to file a motion to modify the court order based upon the lack of cooperation of the other parent. That is more likely to get you where you want to go.
