Visitation Schedule and Agreement
I filed for divorce in California in December 1994. I then moved to Oregon with court approval in December 1996 while my divorce was still pending. My divorce is final, however, on June 30, 1998 I last attended a Family Court Services meeting and a partial visitation agreement was reached. The last paragraph reads:
Parents shall specify a visitation schedule for (child's name withheld) for the summer of 1999 by May 15, 1999. If not resolved, either parent may file a motion to return the matter to Court.
We have agreed on a schedule but this has been a bitter divorce and I believe I should file a copy with the court. I have downloaded every California Judicial form but cannot find what I should be filing. Is there a specific form for a Parental Agreement? Where can I get a sample? Neither he nor I can afford an attorney and I have done much legal work in the past, I'm just not sure what I should file or if there's not a form, what I should entitle the filing using MS Word's template.

If you want the judge to sign it so that it becomes an order of the court, you may have to file it as an "Order Upon Stipulation", with each side
signing a stipulation for the entry of the Order.
Some courts may be more technical and require that you file a Order to Show Cause, and then a Findings and Order After Hearing, with each of the
relevant boxes checked "by stipulation". The Findings and Order After Hearing is the closest "form" which is used.

there is no form. Just call it visitation agreement
