Waiving Parental Rights

Is there a legal document in the State of California, or any other state for that matter in which a parent can waive any future rights for their children?

Example: A friend of mine wants to have her son with a different last name than the fathers, but the fathers last name is already being used. Since the father is known to her, does he need to sign a waiver of parental rights in order for her to have her son's last name changed?

she can file for a name change. she just needs to send notice to the dad. if he does not object it should be granted.
