Where/how do I start to to get custody of children

I am a 36 year old male. My 2 Boys are 11 and 12 and have been living with MY mother for around 10 years...The biological mother has not seen the boys for over 11 years and has abandoned them. They have been with my mother because i was unfit to be a father due to being a alcoholic. and the court has granted MY mother temporary custody. that was over 10 years ago and I have now been sober for 10 years. My mother is getting old and tired of caring for my boys (I have always been there for my boys)and i am now ready to get them. I am getting married in august 2001 and have been raising my fiances boy who is 14 yrs old. No one is contesting that the boys are mine , but i did have a blood test done and the boys are mine. My mother is willing to give them up. So there is no friction there. My question is where do I start? I do not want to hire a lawyer cause i feel that i don't need one since this isnt being contested by anyone...we dont know where the mother is she is a drug addict and has not seen the boys for 10 years and i cannot afford to pay an attorney if i can get the ball rolling myself...Please help with any advice.

Unique file a petition to modify custody. If you want to try do-it-yourself you can buy the forms from the court clerk.
