I have a one year old baby, she has my ex-boyfriends last name, but the real dad wants her to have his last name, is it possible to change her last name

How does a situation like this happen? Is there a declaration of paternity the was signed at the birth? Or is there a judgment of paternity? Or did you just put a name on the birth certificate as the father? What you need to do depends on the answer to these questions.
