My child was born on June 15. When does my 12-weeks of unpaid leave begin? From the date of birth or from the date my actual disability ends?

The 12 weeks permitted is for baby bonding time, which starts when FMLA leave is requested or designated by the employer. Any pregnancy related disability is covered under a separate law, known as the Fair Employment and Housing Act, and can last as long as 16 weeks, if medically supported. The two leaves can be consecutive, assuming you meet the qualifications.
