I need to find case law or a pleading form to do a motion to substitute party in a federal civil matter due to death of one plaintiff.

As far as I am aware, there is no official form in federal court for substition of attorney. You can use the federal forms link on my law firm site to double check.
Off the top of my head, I would say you can use a stipulation for substitution of attorney on standard pleading to substitute one party for another.
