I have am the assignee of a judgment, and want to place a lien on the debtors property in San Bernardino Ca, what forms do I need to do this?

You first need to become the assignee of record. Generally, you file an "acknowledgement of assignment of judgment" with the court clerk. You then have the clerk issue and abstract of judgment and you record that with the county recorder's office in the county where the property is located.
