You need to file and serve a Case Management Statement at least fifteen (15) days prior to the scheduled Case Management Conference. It is form # CM-110. You can find a copy of it on the internet at the following web address: http://www.courtinfo.ca.gov/cgi-bin/forms.cgi

The forms in the right column are fillable, assuming you have the latest version of Adobe Acrobat.
