A case management conference questionnaire is a four page form that is filled out by the parties to a lawsuit, and it must be served on the other party, and filed with the court at least 15 calendar days prior to the Case Management Conference.

It is now known as a Case Management Statement, and is a mandatory form number CM-110. You can find a fillable form online at http://www.courtinfo.ca.gov/cgi-bin/forms.cgi.
