Name Change

I have been using a different name other than my birth name for the past five years under the usage method.I would like to know where can I find a declareation of legal name change form and which department of the Los Angeles court system can I find any forms related to name change?

Go to the downtown court on Hill Street,walk into the clerk's office, and ask them the same question. They'll give you the form and let you know about the requirements. It will be easy.

You can find the forms you need for a name change at accesslaw under the section on judicial council forms. look at the forms in the section based on use. then file them with the court the clerk will send them to the correct department.
