Write up in pleading form, with the case caption, lines and numbers, etc., a summary of your case, your damages, provide evidence including your declaration under penalty of perjury, file the form request for entry of default and lodge a proposed order, and hope for the best! Or, you can have a lawyer assist you in preparing the request. The harder part is trying to collect once you get a judgment! Good luck.
