Deposition subpoena served. Missing address of deponent...

Does this make the subpoena invalid?

I am a party in this case (propria persona) but NOT the witness named in the subpoena. The copies sent to me by the Deposition Officer (Plaintiff's attorney) have a BLANK Proof of Service of Deposition Subpoena for Production of Business Records. No date, names, signature....

Does this make it invalid? As a party, do I need to get a copy off their POS or does that only get sent to the actual witness?

Why are you involved in a case in propria persona?

The blank proof of service doesn't make the deposition subpoena invalid. Probably this is one of those copying services that wants to get your medical records.
