What is a Dismissal of DOES

I am the Plaintiff in a Civil case regarding a Breach of Contract. I have served a summons to the Defendant and he has not responded. I submitted an Entry of Default with a Proof of Service, a Declaration, and a Request for Judgement. Apparently, all I need now is to submit a Dismissal of DOES. I picked up a Request for Dismissal from the courthouse, but don't know how I should fill it out. I don't want to fill it out incorrectly and have the case dismissed as a result. Can anyone walk me through filling it out correctly?

All you need to do is check "dismiss complaint" "without prejudice" and under "other", which you also check, type "Complaint as to Does 1 to ___ (whatever number), only". Of course, fill in all of the other case information at the top of the form, and sign the form where required.
