Legal Question in Civil Litigation in California

I have a fed. proof of service to fill out. A corporation was served, I checked box 4. d on form CV-01, where am I supposed to list the date and time of service on this form? Thank you.

First of all, Form CV-01 is no longer in use. You should use Form POS-01 (which should be available on the court's website). As for the date and time of service, in goes in Section 5 (either subsection "a" or "b" depending on how you effectuated service).

I hope you are using that form in a federal case. If you are using it in a state case, you are using the wrong set of forms altogether.
