Filing fee waiver forms for court filings are available at the following link: http://www.courts.ca.gov/forms.htm?filter=FW

Those forms can be filled out at home, and printed out.

I'm not sure what you mean by a filing fee waiver for the Beverly Hills Bar Association. If you're filing a first pleading with a court, you can find the fee waiver application at http://www.courts.ca.gov/documents/fw001.pdf . There are other forms that are relevant at the courts' website, including instructions and the order approving the fee waiver.

My guess is that you're filing a fee arbitration petition against a lawyer who used to represent you. Bar associations administer those arbitrations, and there are filing fees which can sometimes be waived. The BHBA has a website; that's where you should look. If the form isn't there, call them and ask for it.
