There is no statewide specific form. An Application for Permision to Present a Late Claim (Gov't Code, sect. 911.4) and a Petition for Leave to File Action Against a Public Entity are prepared on pleading paper. I suggest you get an attorney to help you and discuss your case, as you may be wasting your time.
Judges and the court system have a certain degree of immunity from suit; if you do not like a decision that was rendered by a judge you probably are not going to be able to sue the judge or the court system so a claim would be a waste of time. if you post the details of your claim on this site, an attorney may be able to tell you if you have any legal basis for a suit.
