Finding legal forms

Hi. I am involved in a civil case. I owe $11,000 to a creditor. I missed my appearance due to health and am trying to find the forms to the file. I can find templates, but I want to see actual documents filed in similar cases. The docs I need are: 1) affadavit, 2) motion to set aside the default, 3) memorandum of Points & Authorities, and 4) a proposed responsive pleading. If I cannot find samples, are there low-cost services I go to to have them prepare these docs for me?

I cant know if you can set aside the default until I would know the facts. The templates for doing so are less important than your basis for setting aside the default. This is an issue I have fought about in court.
