Form for demurrer

I want to file a demurrer at the superior court of San Francisco. Is there a specific form for demurrer?

No, but you might find samples at a law library. You not only have to provide a notice of demurrer, but a separately stated demurrer as to each objectionable cause of action, and then your points and authorities. See, generally, Code of Civil Procedure section 430.10.
