Is there a form on Small Claims Court to recover the money that was levy from a bank account and latter the judge declares the case vacated?

Someone mentioned "Motion for restitution" but i don't find that form in California's Court.

How to recover that money from court if case was declared vacated?

There is no form. It is a motion that is prepared on pleading paper. You may want to find an attorney to assist you, or speak to the small claims advisor's office in your area.
