Legal Question in Civil Litigation in California

According to the information I have read, I will need to take to Civil court with me a sworn statement from a neighbor. Is a sworn statement "sworn" because it is notorized or does this person need to swear in front of a employee of the court?

A "sworn statement" means to sign it under penalty of perjury. The language should be: "I declare under penalty of perjury under the laws of the State of California that the foregoing is true and correct. Executed this ___ day of September 2009 at ___________, California." That should be followed by the neighbor's signature and the neighbor's printed name beneath the signature.
