If Plaintiff files the original complaint using the court's fillable forms, and then amends the complaint, does Plaintiff have to use the court's fillable form for the amended complaint or can Plaintiff use Plaintiff's personal Microsoft Format version for the amended complaint.

The official complaint form is optional. An amended complaint does not have to use that form, even if the original complaint did. But there are rules governing the format of custom-drafted documents. I can't say whether this particular "personal Microsoft Format version" will meet those requirements, since you haven't described the format.
