You don't need a particular format. Just make sure your document has the case name and number and it provides your position and prior settlement discussions.

You should follow the captioning format used in California Courts as far as putting your name and address on it, the name of the court, case name, case number, etc. Other than that. You also need to read the California Rules of Court sections on Mandatory Settlement Conferences for the list of things you have to cover in the statement (look for the rules on courtinfo.ca.gov) and also any local rules which you will find on the local court's website. Other than that, every settlement conference statement is a document typed up for the specific settlement conference.

I disagree with Mr. Tashijian. As Mr. McCormick points out, there is a format of things that should be covered in a Mandatory Settlement Conference brief. The required contents are set forth in California Rules of Court rule 3.1380, which you can read here: http://www.courts.ca.gov/cms/rules/index.cfm?title=three&linkid=rule3_1380

Additionally, please be aware that some local rules for the county where your case is pending may have additional requirements governing what needs to be submitted.
