Proofs of Service

I am a medical and legal transcriptionist out of my home office. I drafted some documents for a local attorney and he asked me to serve them on the opposing attorney via fax from my home. On the proof of service I declare that I am over the age of eighteen (18) years and not a party to the within action.

Do I state that I am employed at, (a) my home office address, or (b)the address of the attorney for whom I am preparing the document?
