Legal Question in Civil Litigation in California

Hello. I recently been issued a temporary restraining order called "civil harrassment without violence". I am going to court in July to fight it, as it's bogus claims. I've have individuals that are willing to declare that what is being claimed about me...said about me is false. Anyhow, the form they're going to fill out is "Declaration" MC-30 for California. I'm repersenting myself so I dont have an attorney. My question is, is there anything specific about this form I need to know? The first box, do I put my name and address or theirs? I'm going to have them fill the form out by hand and sign it. Do they need to put their address or phone number in their Declaration? And do the courts usually (Ventura) accept it? I know...a lot of questions hehe. Any help would be most helpful. Thank you

You are far better off getting an attorney to assist you and having the witnesses testify in person. Judges discredit written declarations.
